ENERGY EXCHANGE MEMBERSHIP APPLICANT NOTICE

IMPORTANT NOTICE TO ALL APPLICANTS:  In order to ensure compliance with federal or state law, Energy Exchange Listing Service, LLC ("the Service”) must be reasonably satisfied after making reasonable inquiry that each Applicant, or his representative, has knowledge and experience in financial, business matters or technical experience in the petroleum industry, that he is (or they together are) capable of evaluating the merits and risks of participating in oil and gas exploration and development.  This questionnaire is designed to provide the Service with the information necessary to make a determination of whether the Membership Applicant satisfies these suitability requirements.  The information supplied in this questionnaire will be disclosed to no one without the consent of the Membership Applicant, other than (i) the Service, its owner, Energy Exchange, Inc., and employees, agents, accountants and counsel of the foregoing, (ii) state and federal authorities, if deemed necessary by the Service, its owner, Energy Exchange, Inc., or any sales agent of the Service to use such information to support any applicable exemption from requirements of federal or state law, if applicable, and state law which it claims exemption therefrom, or (iii) other regulatory organizations.
